Active decay and gum disease are treated before aligners begin, and that pre-treatment is a separate cost most patients have not budgeted for.

The reason is straightforward. Moving teeth applies steady force to the bone and ligament holding them in place. If that tissue is inflamed and infected, you are moving teeth through compromised support, and you can lose attachment you will not get back.

When I evaluate a new aligner patient, the periodontal exam measures six points around every tooth. Pockets deeper than 3mm point to disease that needs handling first. Sometimes that is a single deep cleaning. Sometimes it is quadrant-by-quadrant treatment of the periodontal problem first before we discuss trays at all. Untreated cavities get restored on the same principle.

So when you compare quotes, ask whether the number assumes a clean bill of health. Most do.
